Commonwealth Health and Rehab Center
The Commonwealth Health and Rehab Center is a combined rehabilitation center and long-term care facility in the City of Fairfax.
The facility is managed by Commonwealth Care of Roanoke, which purchased the former Inova Commonwealth Care Center in October 2010.[1]
